Forms day – August 21st

 

 

All families are required to complete school registration by returning the forms listed below to school on Thursday, August 21st, from 6:00-8:00 pm. Please make sure you print out the forms and fill them out neatly prior to attending.

 

If you provided the school with the Family Registration * & Student Enrollment* form in June, you do not need to provide another, but you must be sure to check in at the registration desk upon arrival. Many forms sent into the school were incomplete or illegible.  Since these contain your child’s emergency information, it is imperative that they be complete.

 

STUDENT’S WITHOUT THE ABOVE  * INFORMATION COMPLETED WILL NOT BE ALLOWED TO ATTEND THE 1ST DAY OF SCHOOL UNLESS ACCOMPANIED BY AN ADULT WHO CAN PROVIDE IT.

SPORTS WAIVERS AND PHYSICAL ARE NOT AVAILABLE ONLINE. PARENTS MAY PICK THESE UP AT SCHOOL ON AUG. 21ST, DUE BACK BY SEPT. 12TH.
